Personal Information: 

  • Legal Name: As per the CRA account. If you changed your name in 2022, indicate it. 
  • Social Insurance Number: Your Canadian SIN or ITN. 
  • Gender & Date of Birth 
  • Contact Details: Phone number, email address, and mailing address (in English for foreign addresses). 
  • Marital Status: As of 2022/12/31. If changed during the year, provide details. 
  • Residence: Information on any changes in residence for tax purposes in Canada. 
  • Citizenship: Are you a Canadian citizen? If yes, do you wish to share your information with Elections Canada? 
  • Spouse/Common-law Partner Information: Full legal name, date of birth, SIN, and net income. 
  • Dependents: Names, SINs, dates of birth, and relationships. 
  • Foreign Property: Do you own foreign property/assets worth more than CAD 100,000 in 2022?

  • T4: Employment or commission income. 
  • T4E: Employment insurance benefits. 
  • T4A: Pension, retirement, annuity, and other income.
